Causes of Asphalt Pavement Deterioration

It is a mandatory requirement that any visitor to a Superior Road Products work site shall complete the Covid-19 Screening Tool Questionnaire below. This must be completed prior to attending the work yard, mobilization yard, or any of our construction sites. All Municipal Partners, Suppliers, or Subcontractors will be required to complete the Questionnaire if they plan to visit one of our sites.

 

Screening should occur before or immediately upon arrival. This screening tool can be completed online before going to the site or on-site before interacting with any Superior Road Products Employee. Superior Road Products will ensure that the result of screening is used to determine whether the visitor may enter the workplace. A visitor may only enter the workplace if they have passed the screening questionnaire, and have reported their presence to Jon Levesque, Operations Manager, at 519-889-1275.
